The Price of a Cute Face
We bred dogs to look like babies — short noses, big eyes, rounded skulls — and gave them lungs that can't breathe. The brachycephalic welfare crisis is hiding in plain sight behind a viral aesthetic.
The French Bulldog became the most registered breed in the UK in 2018, in the United States in 2022, and has topped charts in Singapore and Australia. Across the world, a dog with a face squeezed flat, a skull too round for its brain, and a soft palate draped across a narrowed airway became the defining dog of the decade. It happened because we found them irresistible. And because we found them irresistible, we did not look closely at what we had done to them.
Brachycephalic obstructive airway syndrome — BOAS — is not a fringe condition in flat-faced breeds. It is the structural consequence of the anatomy we selected for. The flat face that looks like a baby's face, that triggers the same neuro-optical response that makes us want to nurture infants, is produced by compressing a normal-length soft tissue structure into a shortened skull. The nostrils are narrowed. The soft palate is elongated relative to the space available. The trachea may be hypoplastic. The nares — the outer nostrils — may be stenotic. The result is a dog that, in many cases, cannot breathe normally at rest, let alone during exercise or in warm weather.
The anatomy of a welfare crisis
To understand BOAS, it helps to think of it as an engineering problem. Evolution produced dogs with skulls of a certain length, soft tissue structures that fit that skull, and airways calibrated to the whole. Selective breeding — accelerated in recent decades by the popularity of extreme conformation — compressed the skull while the soft tissue changed more slowly. The result is a dog whose oral and pharyngeal anatomy is crammed into a space too small for it.
The Cambridge BOAS Research Group, led by Jane Ladlow and Nai-Chieh Liu at the Royal Veterinary College and University of Cambridge, has been the most systematic research programme investigating BOAS prevalence and severity. Using whole-body barometric plethysmography — an objective measure of airway obstruction — they assessed large populations of French Bulldogs, English Bulldogs, and Pugs. Their findings, published in PLOS ONE and elsewhere, found that a majority of the dogs tested showed clinically significant flow-limited breathing. Many of the owners of those dogs reported their pets' breathing as normal.

The normalisation of suffering
This normalisation — the cultural encoding of respiratory distress as a personality trait — is central to the welfare problem. The snoring is "adorable." The snorting is "characterful." The sleep apnoea, the exercise intolerance, the heat sensitivity, the gagging after meals — these are documented by owners on social media as endearing quirks. Veterinarians in BOAS research have a word for this phenomenon: the owners do not know what a healthy dog breathing normally actually sounds like, because they have never owned one.
The welfare implications are serious. BOAS is a chronic condition. It is present every moment the dog is alive, and it worsens with obesity, heat, and excitement — conditions that are, for dogs, daily realities. In severe cases, dogs can experience respiratory crises triggered by nothing more than walking up stairs. Surgical intervention — soft palate resection, nares widening — can improve quality of life significantly, but surgery is not a cure for an anatomy that was never correctly designed, and outcomes are variable.
These figures represent dogs assessed with objective whole-body barometric plethysmography, not owner reports. Owner-assessed prevalence is substantially lower, reflecting widespread normalisation of BOAS signs.
The show ring's role
Dog shows are not simply beauty pageants; they define the standard of the breed. Judges award points for conforming to a written description, and breeders breed toward what wins. For decades, in several flat-faced breeds, winning conformations grew progressively more extreme: flatter faces, more compact skulls, more pronounced wrinkling. The written breed standards themselves have, in some cases, described features now understood to be incompatible with normal health.
Several kennel clubs have moved to address this. The UK Kennel Club revised the breed standard for the Bulldog in 2009 and has issued guidance on brachycephalic breeds. The Royal Dutch Kennel Club (Raad van Beheer) took stronger action: in 2019, following a Dutch court ruling, it prohibited registration of dogs with extreme brachycephalic features. The Netherlands became one of the first countries to enforce welfare-based breeding restrictions with legal backing.
The social media accelerant
The brachycephalic boom of the 2010s and 2020s did not happen in a vacuum. It was amplified by social media, by the visual logic of platforms that favour the cute over the contextual. A French Bulldog with enormous eyes and a flat face photographs exceptionally well. It fits the square frame. It triggers neotenous responses — the same instinct that makes us find human babies appealing — more reliably than a Labrador or a Greyhound. The algorithm does not show the veterinary bills, the respiratory distress, or the surgical report.
Research published by the Royal Veterinary College's VetCompass programme — which aggregates clinical data from UK veterinary practices — has tracked the surge in brachycephalic registrations and the corresponding increase in BOAS-related veterinary interventions. The data illustrates a direct line between aesthetic fashion and clinical workload: as French Bulldog registrations rose, so did the caseload of brachycephalic surgery in referral practices.
What responsible ownership looks like now
The ethical question for prospective flat-faced dog owners is not whether to love these animals — they are already here, in vast numbers, and they deserve excellent care. It is whether to continue generating demand for the extreme conformations that cause suffering. The ICECDogs (International Collaborative on Extreme Conformations in Dogs) consortium, which brings together research teams from the UK, Netherlands, and elsewhere, has published recommendations for buyers: if the nose is invisible from the front, if the dog snores at rest, if it cannot tolerate a short walk without distress, those are signs of BOAS that should prompt veterinary assessment, not social media content.
For those who already own brachycephalic dogs, the most important step is an honest conversation with a veterinarian about airway assessment. BOAS grading tools exist; early surgical intervention, where indicated, produces significantly better outcomes than waiting for a crisis. Weight management is critical: obesity dramatically worsens airway obstruction in already-compromised dogs. Avoidance of heat stress — particularly relevant in Singapore, where ambient temperatures can reach 32–35°C — is not optional but essential.
Beyond the cute face
There is a version of this story in which we look back on the brachycephalic boom the way we now look back on other welfare failures that were hiding in plain sight — cosmetic procedures on animals, the use of performance-enhancing drugs in racehorses, the casual acceptance of docking and cropping as aesthetic norms. The practices did not end because people suddenly became cruel; they ended because someone named what was happening clearly enough that the culture could no longer avoid it.
The price of a cute face is paid in breath. Not by the people who choose the face and bring the dog home. By the dog, alone in the heat, fighting for air through an airway that was never built to let it breathe properly. That is the trade we make when we buy the aesthetic. The least we can do is know we are making it.
