Heartworm in Tokyo dogs: what the humid summers mean for your prevention schedule
Tokyo's June–September monsoon season creates one of Asia's most intense mosquito windows — and heartworm risk comes with it. Many expat dog owners arrive from cooler climates and dangerously underestimate what that means for their prevention calendar.
The first summer in Tokyo has a way of resetting every assumption you had about climate. The tsuyu rainy season arrives in early June, and by July the city has settled into something close to a subtropical steam bath — humidity regularly above 80%, temperatures hovering at 30–35°C, and a mosquito population that comes alive with spectacular density in parks, riverbanks, and the dense residential neighbourhoods in between. For dog owners, this is not just uncomfortable. It is the precise environmental window during which Dirofilaria immitis — the heartworm parasite — completes the larval development inside its mosquito hosts that makes transmission to dogs possible.
Tokyo sits at approximately 35°N latitude, which places it closer to heartworm-endemic tropical and subtropical zones than most Western European or North American cities. Studies across Japan confirm that heartworm has been detected in domestic dogs across the archipelago, with higher prevalence in warmer, more humid regions including the Kanto plain where Tokyo is situated. The Japan Animal Welfare Society and Japanese veterinary literature have consistently documented that prevention — not treatment — is the correct clinical response, and that monthly preventive medication is the standard of care for dogs in Japan throughout and beyond the warm season.
How heartworm transmission actually works — and why Tokyo's climate matters
Dirofilaria immitis is a filarial nematode transmitted exclusively through the bite of an infected mosquito. The life cycle requires a mosquito host: when a mosquito bites an infected dog and ingests microfilariae (immature larvae circulating in the blood), those larvae undergo a critical development phase inside the mosquito before reaching the infective third larval stage (L3). The key biological constraint — and the reason climate is central to heartworm risk — is that this larval development inside the mosquito only proceeds when ambient temperatures are consistently above approximately 14°C (57°F). Below that threshold, development stalls.
In Tokyo, average temperatures exceed 14°C from roughly April through October — a six-month window. The peak transmission risk, however, aligns with the hottest and most humid months: July through September, when the development of L3 larvae inside mosquitoes is rapid, mosquito activity is at its annual maximum, and the urban population of Culex tritaeniorhynchus and Aedes albopictus (the tiger mosquito, highly urban and an aggressive daytime biter) reaches its seasonal peak. Aedes albopictus in particular has expanded its range across Tokyo's wards over the past two decades and is now a year-round concern in warmer microclimates near the city centre.
The mosquito species involved in Tokyo
Two species are of primary clinical relevance for heartworm transmission in the Tokyo metropolitan area. Culex tritaeniorhynchus is a night-biting species with strong presence near agricultural water, rivers, and the many canals and drainage channels that crisscross the city's outer wards (Edogawa, Katsushika, Adachi, Koto). It has historically been one of the dominant heartworm vectors across Japan. Aedes albopictus — the Asian tiger mosquito — is a daytime biter that has colonised urban Tokyo aggressively and is now found in residential gardens, parks, and planters across the 23 wards. Unlike Culex, it breeds in tiny amounts of standing water: a saucer under a plant pot is sufficient. Both species are capable of transmitting heartworm if they carry infective larvae.
The prevention window: why Tokyo dogs need an earlier start than many Western owners expect
The standard WSAVA and American Heartworm Society (AHS) guideline for temperate climates recommends beginning monthly prevention approximately one month before the start of mosquito season and continuing for one month after the first frost. In Tokyo, that translates to a start date no later than the end of April, and continuation through at least the end of November — with many Japanese veterinarians recommending year-round dosing given the urban heat island effect that keeps temperatures in central Tokyo elevated relative to regional averages, and the increasing unpredictability of autumn cooling.
Expat dog owners who arrive from the UK, Scandinavia, Germany, or northern North America frequently underestimate this window. In London, heartworm is not a meaningful concern. In Vancouver, the risk window is narrow. In Tokyo in mid-summer, a dog that has been off prevention for even two months is genuinely exposed during peak transmission season. Japanese veterinarians report that missed or delayed prevention — especially in rescue dogs adopted without a clear parasite history — is one of the more common preventable scenarios they manage.
Prevention products available from Japanese veterinarians
In Japan, heartworm prevention products are prescription-only (要指示薬 — yō shiji yaku) — they cannot be purchased over the counter at pet shops or online without a veterinary prescription. This is a meaningful difference from the regulatory environment in some countries where prevention products are available OTC, and it means your first step in any prevention protocol in Tokyo is a veterinary consultation. Japanese veterinary clinics (動物病院, dōbutsu byōin) near residential areas in every ward stock approved prevention products; you do not need to import from abroad.

Before starting prevention: the heartworm test
Any dog that has had a gap in prevention — whether due to relocation, a lapsed schedule, or adoption from an unknown background — should be tested for existing heartworm infection before beginning or resuming prevention. Japanese veterinary clinics can run an antigen test (for adult female worm proteins) in-clinic in approximately ten minutes, or send a sample to a diagnostic laboratory. The test detects infections with adult female worms; infections acquired within the previous five to six months (the pre-patent period) may not yet be detectable, so a retest six months later is sometimes recommended. A positive test result requires a treatment decision — which in Japan follows similar protocols to those used internationally, including an adulticidal course under close veterinary supervision.
Signs of heartworm disease to know
Early heartworm infection is frequently asymptomatic, which is precisely what makes it dangerous and why prevention rather than detection is the clinical priority. As the worm burden develops and adult worms establish in the pulmonary arteries and right side of the heart, clinical signs emerge progressively: a mild persistent cough (often the first sign owners notice), exercise intolerance or reluctance to play, fatigue after moderate activity, reduced appetite, and weight loss. In advanced infections, a pot-bellied appearance from fluid accumulation, fainting or collapse, and signs of heart failure may appear. If you observe any of these signs in your dog, see a veterinarian promptly — do not attempt to adjust or begin prevention medication without professional guidance.
Practical advice for Tokyo dog owners
- Find a Japanese veterinarian before your dog needs one — registration at a local dōbutsu byōin is part of settling in. Use the clinic to get your heartworm test done and your prevention protocol established in the same appointment.
- Start prevention in late April at the latest. Year-round dosing is increasingly the recommendation for urban Tokyo dogs given the city's heat island effect and the extended warm season.
- Set a monthly reminder — missed doses are the most common reason prevention fails. Many Japanese clinics offer an annual reminder service for prevention refills.
- Eliminate standing water on balconies and in gardens during the summer months. No mosquito breeding means fewer bites.
- Dogs walking along the Arakawa, Tamagawa, and Edogawa river corridors, or in the outer wards with more agricultural land, have somewhat higher mosquito exposure than those confined to the dense commercial cores — factor this into your vigilance.
- If travelling with your dog within Japan, note that Okinawa and other southern prefectures have year-round heartworm transmission risk and generally higher local prevalence.
